Declaration of Interest Form for parish and town councillors (PDF 223KB / 8 pages)

Councillors must include their home address, as well as details of any other land or premises they have a financial interest in section 4 of their declaration interest form.

Addendum Gifts and Hospitality Form (PDF 66KB / 1 page) must be used if a parish or town councillor has completed a Declaration of Interest form and wishes to declare a new offer.

Addendum Other Registerable Interests form (PDF 73KB / 2 pages) must be used to declare 'other registerable interests', for example, appointments to outside bodies.

Councillors don't need to complete a new declaration form if they complete an addendum form, unless the change is to do with their 'registerable pecuniary interests' (ie the first part of the Declaration of Interest form, which covers the legal requirement to disclose). In this case, the councillor must complete a whole new Declaration of Interest form.
